Information for Reimbursement
The following information is provided to assist you in preparing
information and receipts for reimbursement. Please submit the
appropriate information and receipts no later than Oct. 15, 2006. Receipts
should be mailed to Joanne Fife at the address indicated below.

Your Reimbursement
In order to efficiently process your reimbursement, we be requesting
the following information from you. Please use this form (pdf) to submit your reimbursement request.

- An itemized list of expenses to be reimbursed. If you are
claiming expenses associated with use of your personal vehicle,
provide your vehicle license plate # and a statement confirming
you have the required liability insurance.
- Original receipts for all expenses over $25 to be reimbursed
(or partially reimbursed).
- Legal First and Last Name (should match Social Security card)
- Home address
- If not a U.S. citizen or permanent resident, please indicate
VISA status.
Additional Information for International Travelers
In order to be reimbursed for workshop expenses, international travlers
must obtain the appropriate immigration status from U.S. Citizenship
and Immigration Services (USCIS). Please note that a passport
alone is not sufficient evidence to determine immigration status
and must be used only in conjunction with the appropriate USCIS forms
(I-94; I-551) to appropriately identify immigration status.
To be reimbursed, international travelers must travel by U.S. flag
air carriers. There are limited exceptions. If U.S. Flag
Carrier Services are not available, please indicate such so that
we can request an exception to this restriction.
